SUMMARY: Responsible for the coordination and final review of the MDS and Care Planning Process, utilization review, selected Quality Improvement studies and the Resident Tuberculosis Program.  As part of the nursing manager team, participates in the on-call schedule and early AM staffing.

Compensation: is $44/hour.

ESSENTIAL DUTIES and RESPONSIBILITIES:  include the following. Other duties may be assigned.

Coordinates and performs review of the Minimum Data Sheets and care planning process. Coordinates benchmarking for quality improvement. Coordinates selected Quality Improvement studies and the Resident Tuberculosis Program. Participates in leadership meetings and on call-scheduling. Coordinates managed care resident insurance certification and recertification. Provides education to staff regarding documentation. Audits clinical records and compiles statistical data on skilled residents. Assists with, coordinates, and attends utilization rounds; provides information during meeting for skilled services plan of care, discharges and resident status updates. Coordinates the development of a written plan of care (preliminary and comprehensive) for each resident. Develops and monitors nursing procedures to ensure that supplies are used in an efficient manner to avoid waste. Ensures all resident assessments are completed in a timely manner and in accordance with regulatory standards. Ensures documentation on resident certification for skilled nursing services is completed and maintained within Standard Federal Regulations. Collects and analyzes QA data as assigned. May review and complete the CMI process. May enter and review all new admissions diagnoses in Visio. May participate in Nurse Leadership Team activities.

EDUCATION and/or EXPERIENCE:

  • Minimum of 2 years’ experience performing MDS in long-term care preferred.
  • Minimum of 2 years’ experience as part of a nurse leadership team in a LTC preferred.

LANGUAGE SKILLS:

Must be able to read, write, understand and communicate effectively in the English language.

LICENSES, CERTIFICATES, REGISTRATIONS:

  • Current license as a Registered Nurse in the state.
  • Current CPR certification.

  • OSHA OCCUPATIONAL EXPOSURE CATEGORY: After careful analysis, it has been determined that this position falls into OSHA Occupational Exposure Category III and requires the following protective equipment be worn by anyone filling this position.
  • GLOVES, GOWNS, MASKS, AND/OR GOGGLES: Training will be provided in how to properly and effectively use any necessary equipment, in addition to education regarding precautionary measures, epidemiology, and modes of transmission and prevention of HIV/HBV.

ROUTINE DECISION-MAKING:

  • Ensuring accuracy of data entered in the MDS sent to the state.
  • Making decisions regarding residents’ care plans.
  • Other routine decisions as they pertain to the essential functions of the position.

FORMAL POLICY-SETTING RESPONSIBILITIES:

  • May make recommendations regarding policies.
