Home Care Physical Therapist FT - This team member is responsible for providing a holistic approach, supporting the residents' physical, spiritual, emotional, and clinical needs. He/she is also responsible for implementing residents’ personal care plans, focusing on residents’ strengths, preferences, and preferred routines, while providing a sense of comfort, companionship, and belonging.

Rehab Therapy Tech - The team member in this physical therapy role will make a difference by ensuring the resident care policies and procedures are followed in compliance with applicable quality of care standards. He/she is also responsible for assisting clinical rehabilitation staff with the preparation of treatment area, customer transport (ambulatory or wheelchair) to and from the Rehab department.

Outpatient Physical Therapist - Team members in this role support our residents in all aspects of daily living, providing person-centered care to ensure we meet all of our resident’s needs.
