Licensed Social Worker - This team member is responsible for providing social work services to the residents and family members in skilled nursing, long-term rehab, assisted living, and memory care. He/she completes psychosocial assessments, MDS, and other required documentation in accordance with department policy, State, and Federal regulations.

Resident Services Coordinator - This team member is responsible for conducting assessments of potential residents and recommending appropriate level of residency. He/she is also responsible for providing psychosocial support to residents and families and assisting through the transition process to higher levels of care.

Resident Services Coordinator II - This team member will bring their passion for resident engagement into our community. He/she will make an impact by responding to residents in need of psychosocial or supportive services, through direct referral or by the review of incident reports.
